Case Management

Our case management team serves as the first point of contact for individuals and families seeking support. Whether through walk-ins or referrals, we take time to understand each person’s unique situation and assess their needs holistically.

Where possible, we connect clients to our in-house programmes and services. If their needs fall beyond our scope, we work closely with community partners to ensure they are referred to the appropriate support network.

Eligibility Criteria

  1. Singapore Citizens and/or Permanent Residents (PR). At least one member of the family has to be a Singapore citizen.
  2. Residing within Jurong Central SMC and neighboring estates
  3. Monthly household income between $3000 to $4200 or Per Capita Income between $800 to $1000^.

Upload the following documents together with the application form:

  1. Identification Documents (e.g., NRIC, FIN)
  2. Recent Payslip or CPF statement for relevant household members
  3. Student pass/ concession (for Children/Youth programmes)
  4. Medical Reports (if applicable)
  5. Chas Card (if applicable)
^Application and type of assistance will be assessed and determined on a case-by-case basis.
